The Centers for Medicare & Medicaid Services (CMS) has resumed and increased in-person site visits for provider enrollment verification throughout 2025, including a surge in unannounced inspections following the post-COVID suspension. These visits target practice locations listed in the Provider Enrollment, Chain, and Ownership System (PECOS), confirming operational status, compliance with licensure, and alignment with enrolled specialties. As of December 2025, CMS has conducted tens of thousands of visits nationwide, focusing on high-risk areas like home health agencies and independent diagnostic testing facilities, but extending to physician offices in primary care and surgery. Activities were paused during the October-November 2025 government shutdown but have resumed fully. 

Site visits entail CMS contractors physically inspecting addresses for signage, active patient flow, and record-keeping, with discrepancies triggering enrollment holds or revocations. Providers must ensure PECOS records reflect current physical addresses, including those for telehealth-only practices added under 2025 expansions. Updates via PECOS involve logging into the Identity and Access Management System, navigating to the "Manage Enrollment" section, and submitting Form CMS-855I changes, which process in 30-60 days. Failure to verify locations can result in payment suspensions, as evidenced by 2025 data showing audit flags leading to withheld reimbursements. 

New PECOS location types introduced in July 2025 include "Provider-Based Department" for hospital-affiliated sites, aiding accurate classification and reducing audit risks. Providers are required to report changes within 30 days, per 42 CFR § 424.516, with non-compliance penalties up to $10,000 per violation. The resurgence aligns with CMS's fraud prevention efforts, amplified by the 2025 PFS final rule emphasizing site integrity amid a 2.93% payment cut. 

Preparation involves cross-checking addresses against state licensing boards and uploading proof like utility bills or leases during updates. Novitas Solutions and Palmetto GBA, Medicare Administrative Contractors, offer webinars on PECOS navigation, stressing integration with CAQH for synchronized data. In 2025, unannounced visits have risen significantly, targeting urban areas with high claim volumes. Providers documenting reasons for location changes—such as relocations due to lease expirations—can appeal audit findings through the Administrative Law Judge process. 

This verification process supports Medicare's goal of ensuring accessible care, with 2025 reports indicating improved network accuracy post-visits. Behavioral health and rural providers face additional scrutiny under expanded telehealth rules, requiring hybrid location listings.
