UnitedHealthcare Streamlines HCBS Provider Onboarding Process 


UnitedHealthcare Community Plan has updated the onboarding process for Home- and Community-Based Services (HCBS) providers, effective as of May 2026. The changes apply to providers supporting Medicaid and long-term services and supports (LTSS) programs and are designed to reduce delays, eliminate misrouting, and simplify the overall enrollment experience. 


Previously, HCBS onboarding requests were frequently delayed due to routing issues and unclear submission instructions. The revised process addresses these pain points directly: the request form has been redesigned to align with specific HCBS service types, routing is now automated to reach the correct team, and instructions have been clarified to avoid the need for resubmission. 


The updated process covers a broad range of HCBS service categories, including personal care and in-home support (personal care, attendant care, homemaker, habilitation, respite, private duty nursing), case management and supports coordination, residential settings such as assisted living and adult foster care, adult day and community-based services, and self-direction supports including fiscal intermediary services, home-delivered meals, personal emergency response systems, assistive technology, and home and environmental modifications. 



How to submit a participation request under the new process: 


  1. Visit the UnitedHealthcare Provider Portal and navigate to Join our network – Ancillary providers and centers 
  2. Select Home- and Community-Based Services (HCBS) 
  3. Complete and submit the online request form and retain your confirmation number 

After submission, UHC will review the application and issue a decision within 15 business days. If approved, a representative will contact you with next steps. If the application is not approved, providers will receive written notification of the determination via email or letter. 


Providers who do not receive a confirmation or encounter issues after submitting can reach UHC support through the 24/7 live chat feature within the UnitedHealthcare Provider Portal.
