Notice for Providers in the New York Network Management IPA: Hamaspik Medicare Plan Updates 

  

If you are a member of the New York Network Management IPA and contracted with Hamaspik, Inc., there’s important information regarding Hamaspik's Medicare plans, which cater to dual-eligible enrollees (those eligible for Medicare and Medicaid). These plans are classified as dual eligible special needs plans (D-SNPs) and include: 

  • Hamaspik Medicare Select (HMO D-SNP) 
  • Hamaspik Medicare Choice (MAP FIDE-SNP) 

As part of the requirements from the Centers for Medicare and Medicaid Services (CMS), Hamaspik is obligated to publicize its Model of Care (MOC). This document outlines how Hamaspik provides healthcare coverage and services to its enrollees. CMS also mandates that providers serving Hamaspik Medicare members read and attest to the MOC. 

 

How to Access the MOC Training: 

You can access the Model of Care training document through one of the following ways: 

  1. Website: Visit Hamaspik's website (www.hamaspik.com/Providers). Navigate to “Resources” and find the “Model of Care Training (MOC)” section. 
  2. PDF Attachment: The MOC training document is attached as a PDF for your convenience. 
  3. Hard Copy: You can request a hard copy by contacting provider relations at providerrelations@hamaspikchoice.org or by calling 845-503-0907

 

Provider Attestation Requirement: 

Hamaspik requests that you make this training available to your practitioners. Once reviewed, please complete the attestation form and return it to melbaum@hamaspikchoice.org by October 30, 2024. Alternatively, you can send it via fax to 845-655-5160. Only one attestation per practice is required.
