Fidelis Care: Annual SNP Model of Care Training Required for All SNP-Treating Providers
Fidelis Care has issued a reminder to providers regarding its Annual Medicare Special Needs Program (SNP) Model of Care (MOC) training requirement. Per CMS regulations, health plans must provide annual MOC education to providers treating their SNP members. This includes both Dual Eligible Special Needs Plan (D-SNP) members, who qualify for both Medicare and Medicaid, and Chronic Condition Special Needs Plan (C-SNP) members.
Per Fidelis Care's provider manual, all providers who treat SNP members must complete this training annually by December 31, regardless of network participation status. This means the requirement applies even to out-of-network providers who render services to SNP-enrolled patients.
How to complete the training:
- Log into Provider Access Online (Fidelis Care's provider portal)
- Watch for a notification on the Home Page if training/attestation is due
- Click the notification and access the module via File Downloads
- Complete the attestation once the training is reviewed
Providers attesting on behalf of a group or organization are responsible for communicating the training content to other providers within that group. The attestation itself is submitted automatically to Fidelis Care once completed.
Why this matters for credentialing teams:
Although this isn't a credentialing action in the traditional sense — it does not affect base credentialing or TIN linkage — it is a compliance requirement tied to servicing SNP members. Credentialing/compliance staff should track completion dates for providers seeing Fidelis SNP members and build a reminder ahead of the December 31 deadline each year.
