CMS Withdraws BOC DMEPOS Accreditation Authority: Immediate Action Required 

Effective: December 2, 2025 | Applies to: All DMEPOS suppliers accredited by BOC 


The Centers for Medicare & Medicaid Services (CMS) has withdrawn approval of the Board of Certification/Accreditation, International (BOC) as an accreditation organization for suppliers of durable medical equipment, prosthetics, orthotics, and supplies (DMEPOS). Effective December 2, 2025, BOC no longer has authority to perform any accreditation activities for Medicare enrollment purposes. 


What BOC Can No Longer Do 

As of December 2, 2025, BOC cannot: 

  • Review suppliers for CMS quality standards compliance 
  • Process pending accreditation applications 
  • Perform reaccreditation activities 
  • Survey or accredit new locations 
  • Survey or accredit for new products 
  • Execute any other functions as a CMS-approved accreditation organization 


Impact on Current BOC-Accredited Suppliers 

Existing accreditation remains valid: If your facility was accredited by BOC prior to December 2, 2025, that accreditation remains valid for three years from the original accreditation date. However, you cannot use BOC for reaccreditation when your current accreditation expires. 

Transition required for: Suppliers who need to: 

  • Renew expiring accreditation 
  • Change facility location 
  • Update ownership information 
  • Add new products to existing accreditation 
  • Modify any other DMEPOS accreditation information 


These suppliers must obtain accreditation from a CMS-approved organization before making any changes or renewals. 


Impact on Prospective Suppliers 

DMEPOS suppliers currently in the BOC accreditation process must immediately seek accreditation from an alternative CMS-approved organization to complete Medicare enrollment. BOC cannot finalize any pending applications, including those already surveyed prior to December 2, 2025. 

CMS-Approved DMEPOS Accreditation Organizations 

The following seven organizations remain CMS-approved for DMEPOS accreditation: 

  • Accreditation Commission for Health Care (ACHC) 
  • American Board for Certification in Orthotics, Prosthetics & Pedorthics (ABC) 
  • Community Health Accreditation Program (CHAP) 
  • Healthcare Quality Association on Accreditation (HQAA) 
  • Joint Commission (JC) 
  • National Board for Respiratory Care (NBRC) 
  • Compliance Team (CT)

Required Action: Update Medicare Enrollment 

Suppliers who obtain new accreditation from a CMS-approved organization must update Medicare enrollment within 30 days by submitting Form CMS-855S:

Additional Context 

This withdrawal follows CMS's November 2025 final rule requiring annual (rather than triennial) DMEPOS supplier reaccreditation effective January 1, 2026. However, suppliers with current three-year accreditations issued before January 1, 2026, will complete their existing cycles before transitioning to annual reaccreditation. 

In June 2025, BOC and CMS had agreed to a three-year suspension of new accreditation and reaccreditation applications in Florida, Texas, California, and New York following misconduct by a former independent contractor. The December 2 withdrawal represents a complete termination of BOC's accreditation authority nationwide. 


BOC has filed a lawsuit (Board of Certification/Accreditation International, Inc. v. Robert F. Kennedy, Jr. et al, Case No. 1:25-cv-04150-MJM, U.S. District Court of Maryland) challenging the withdrawal, describing CMS's action as "abrupt" and stating the agency "refus[ed] to have verbal conversations with BOC before withdrawing BOC's deemed status." 


Important Notes 


  • BOC certifications remain valid: Individual professional certifications (e.g., BOC-certified practitioners) continue to be recognized by CMS, private payers, and state regulatory bodies. This withdrawal affects only organizational accreditation for DMEPOS suppliers. 
  • Medicare billing privileges at risk: Suppliers without active CMS-approved accreditation cannot maintain Medicare billing privileges. Address reaccreditation needs immediately to avoid enrollment complications.
