Centers Plan for Healthy Living Acquisition: Anthem HP Product Transitions 

Effective: January 1, 2026 (MAP) | March 1, 2026 (MLTC) 

 

Elevance Health, Inc. (parent company of Anthem HP, LLC) has acquired Centers Plan for Healthy Living, LLC, triggering product line transitions for Medicaid Advantage Plus (MAP) and Managed Long-Term Care (MLTC) members. 

 

Product Transitions 

Medicaid Advantage Plus (MAP) — Effective January 1, 2026: 

  • Members automatically enrolled in Anthem HealthPlus Full Dual Advantage LTSS 2 (HMO D-SNP) unless they select an alternative MAP plan 
  • Affects dual-eligible beneficiaries (Medicare and Medicaid) 

Managed Long-Term Care (MLTC) — Effective March 1, 2026: 

  • Members transition to Anthem HP's MLTC product unless they select a different MLTC plan 
  • Two-month delay allows staged operational implementation 

 

Member Changes 

  • New ID cards: All impacted members receive cards displaying Anthem HealthPlus Full Dual Advantage LTSS 2 (HMO D-SNP) 
  • Dental benefits: LIBERTY Dental Plan (effective January 1, 2026) 
  • Vision benefits: Superior Vision (effective January 1, 2026) 

 

Provider Credentialing and Billing Impact 

Critical: Current Centers Plan network participation does not automatically transfer to Anthem HP networks. 

Required Actions: 

  1. Verify Anthem HP network status: Confirm credentialing and participation status with Anthem HP 
  2. Update billing systems: Reflect Anthem HP as payer for transitioned members; claims submitted to Centers Plan after transition dates may be rejected 
  3. Verify eligibility: Confirm member enrollment and effective dates beginning January 1, 2026 (MAP) and March 1, 2026 (MLTC) 
  4. Update ancillary referrals: Reflect new dental (LIBERTY Dental Plan) and vision (Superior Vision) vendors for services requiring authorization 

 

Operational Considerations 

Practices serving dual-eligible and long-term care populations should prioritize staff training on Anthem HP billing procedures, authorization requirements, and eligibility verification protocols during Q1 2026. Dual workflows will be required during the staggered transition period.
