Carelon: Removing All CAQH Service-Line Addresses May Be Seen as a Termination

Carelon Behavioral Health has issued a specific warning about the way service-line addresses are maintained in CAQH. 


In its second-quarter 2026 provider newsletter, Carelon explains that removing all Carelon service-line addresses from CAQH may be interpreted as a termination of the provider's practice locations. The issue can therefore affect more than the accuracy of a CAQH profile. Carelon specifically connects the information to network status, claims and payments, contracting and credentialing. The warning is particularly relevant when a provider is relocating, closing a location or changing the structure of a practice. 


Carelon advises providers to maintain at least one active service-line address in CAQH and to contact Carelon before closing or relocating locations. The same guidance says that when a provider adds a service location in another state or under a new Tax ID, the provider must update both CAQH and Carelon directly. 


Carelon's current provider-profile guidance also identifies CAQH as its preferred method for demographic updates and provides separate procedures for changes involving business names, Tax IDs, and other contract information. 


Why it matters for your practice 


The important point is that CAQH is not necessarily functioning as a neutral address book. For a credentialing specialist, deleting an address may look like ordinary data maintenance: the provider no longer practices there, so the old location should come off the profile. 


Carelon's warning shows that the payer may interpret the change differently when that address is the provider's last Carelon service-line location. That distinction becomes particularly important for multi-location practices. Closing one office is not the same thing as ending the provider's participation, and a credentialing team needs to make sure the data change communicates exactly what the practice intends. The same issue applies when a provider expands. A new service location in another state or a new Tax ID may require action with Carelon in addition to the CAQH update. 


What to do — and what not to do 


  • Before removing an address from CAQH, determine whether it is the provider's last active Carelon service-line address. 
  • If the practice is closing or relocating a location, coordinate the change with Carelon rather than relying exclusively on the CAQH update. 
  • If a new state location or Tax ID is involved, follow Carelon's instructions for updating both systems. 
  • Do not remove the last Carelon service-line address from CAQH as a routine cleanup step without first confirming how Carelon expects the change to be handled. 


For Carelon, removing the last service-line address from CAQH may communicate more than “this location closed.” Check the payer's process before making the change.
