CAQH: Removing All Service Line Addresses May Trigger Automatic Termination (Carelon Behavioral Health) 


What Happened

Carelon Behavioral Health has issued a formal warning to all network providers: if a provider removes every Service Line address from their CAQH ProView profile, Carelon interprets this as a voluntary termination of all practice locations. This is not a system glitch or edge case — it is Carelon's stated policy. Because Carelon relies on CAQH as its primary source of provider data, an empty address section signals that the provider is no longer operating at any location, and termination procedures may be initiated automatically.


What Providers Should Watch For

The consequences of unintentional termination are significant and can affect multiple areas simultaneously. Network and panel status may be suspended, claims in process may be impacted, payments may be interrupted, and credentialing and contracting records may be affected. Providers who are simply relocating, temporarily closing a location, or cleaning up their CAQH profile may not realize that removing the last address on file triggers this outcome.

There is also a specific scenario that requires extra attention: if you are adding a new service location in a different state or registering under a new Tax ID, updating CAQH alone is not enough. In these cases, Carelon must also be notified directly — CAQH changes do not automatically flow through to Carelon's internal records for new states or new tax identifiers.


What to Do

The rule is simple: always keep at least one active Service Line address in CAQH at all times. If you are relocating, closing a location, or making structural changes to your practice, contact Carelon Provider Services before touching your CAQH profile. Do not make changes first and notify Carelon after — by then, termination may already be underway.

To verify your current address status, log in to CAQH ProView, go to Provider Data Management, select your provider record, and navigate to the Practice Location (Service Locations) section. If no rows are displayed, Carelon already sees your record as terminated.

Make it a routine practice to check this section during your regular CAQH attestation cycle, especially if multiple staff members have access to your profile and could make changes without full awareness of the downstream effects.
