Anthem Releases Required 2026 Model of Care Training for SNP Providers
Anthem has made the required 2026 Model of Care (MOC) training available for providers participating in Special Needs Plans (SNPs). Completion of this training is a contractual requirement for all network providers serving SNP members and must be completed within the timeframe specified in the provider agreement.
The Model of Care is a CMS-mandated framework that governs how Special Needs Plans — including Dual Eligible SNPs (D-SNPs), Chronic Condition SNPs (C-SNPs), and Institutional SNPs (I-SNPs) — coordinate and deliver care to their high-need populations. CMS requires health plans to train all network providers on the MOC annually. Providers who do not complete the training risk non-compliance with their network participation requirements.
The 2026 training covers the core components of the SNP Model of Care, including individualized care planning, care coordination protocols, the role of the interdisciplinary care team, transition of care processes, and quality measurement goals. The training is intended to ensure that providers understand how Anthem's SNP programs are structured and how to support members with complex medical, behavioral, and social needs.
The training is available through Anthem's Provider Training Academy, accessible via the provider portal at providers.anthem.com. It is offered at no cost and can typically be completed online at the provider's convenience.
Action required for credentialing and compliance teams:
- Confirm that all providers rendering services to Anthem SNP members complete the 2026 MOC training
- Retain documentation of completion — attestation records may be requested during credentialing re-attestation or audit cycles
- Providers who support both D-SNP and non-SNP Anthem members should verify whether separate training modules apply
Failure to complete required MOC training is a common finding in payer credentialing audits and can result in corrective action plans or network termination. This training requirement applies to both participating physicians and mid-level practitioners credentialed with Anthem SNP products.
